CLINICAL SCOPE

Clinical nursing support at home

Complex wound care

VAC therapy, chronic non-healing wounds and post-surgical complications.

Palliative & end-of-life

Symptom management, family support, terminal phase care and communication with the palliative team.

Medication management

Polypharmacy, post-discharge reconciliation, Webster packs and administration under GP authority.

Post-hospital recovery

Wound care, monitoring, medication support and escalation of deterioration.

Ideal referral profile

Needs clinical nursing as the majority of care.

Needs longer visits of 21 minutes or more.

Requires a 1–2 visits per day rhythm.

May qualify for a palliative add-on.

OUT OF SCOPE

High-acuity hospital-substitution care.

Personal care only with no clinical component.

IADLs such as cleaning, shopping or transport.

RN assessment → care plan → ongoing communication.

COMMUNICATION

What happens after referral

We communicate with your practice on receipt within 24 hours, after assessment with a care plan summary, if the condition changes and clinical escalation is required, at regular intervals, and at discharge.

Our goal is to maintain clear communication so your practice is not left chasing updates.
