DVA COMMUNITY NURSING

The DVA Community Nursing Program

The DVA Community Nursing Program supports eligible veterans to receive clinically appropriate nursing care in their own home. Services are based on assessed clinical need and begin following an appropriate referral.

One Vision Community Nursing is a DVA-contracted Community Nursing provider. We are an independent healthcare provider and are not affiliated with or endorsed by the Department of Veterans’ Affairs.

Eligibility and referrals

Access to DVA Community Nursing depends on individual circumstances, referral requirements and applicable DVA eligibility conditions.

Our focus areas

Our nursing service is focused on clinical care that can be appropriately delivered in the community and in the veteran’s home.

Complex wound care

Clinical assessment and management of chronic, surgical and hard-to-healing wounds.

Palliative & end-of-life nursing

Symptom management, family support and clinical nursing during the palliative and terminal phases.

Medication management

Support with complex medication regimens, administration, reconciliation and ongoing monitoring.

Post-hospital recovery

Nursing support following hospitalisation or surgery, including monitoring and escalation when required.
